So.. I've never played online games before and I new at this.. seems like every time I log in I'm in a different "numbered" area.. "Millennium City 28" or "Millennium City 13". Is there a way to pick where you go? Like if I'm in 20 and my friend is in 12, can I switch somehow? And/or is there a way to pick before to log in or something? Thanks.

If you click on the bar with "Millennium City X" it'll bring up a small menu with the option "Change Instance..." clicking on that will bring up a scrollable menu of all the different instances currently running.
If you want to stay in the same instance as your friend then forming a team with them will guarantee that you never get put into two different instances when going to different zones such as Canada, Club Caprice or crafting stores.

As Honorguard said, it's basically just a teaming mechanism. That way, you can easily meet your friend in a specific instance.
Also, it's a way of keeping the game from lagging out - if everyone was in the same instance, the game would be frozen with lag. So basically, instances allow Cryptic to divide up the game into sections with manageable amounts of players in order to keep up game performance.

By default, you don't get to choose where you go while zoning out of instances. It'll automatically distribute you to the ideal place to be. There is an option in the HUD tab of the game options that says "Show Map Transfer Choices."
It's off by default, but if you turn it on, any time you zone into a major zone (Canada, Desert, MC, etc.) from either an instance or another zone, it will present you with the option of where you want to go instead of automatically redirecting you.
