Does anyone else have problems with lunges not always working properly? Sometimes, when I use a lunge, I either don't move at all (yet the enemy still takes damage/effects) or I only move partway to the target. This seems to be particularly common when I've got flight activated and lunge at a ground-level target while being at ground-level myself.
I guess I'm just wondering if this is a bug that needs reporting, or if I just have bad luck, or if the game just hates me.

I get that too. and here I thought it was just some weird lag-effect from my sub-optimal system, like how every once in a while I'd "jerk" backward a step or two when moving.

Flight has always interfered with lunges. It may have something to do with the way that flight gives you movement resistance (to keep you from gliding all over the place after you've let go of the keys).
For example, notice how while flying, knockbacks seem to have less influence on you, such that you'll quickly come to a stop rather than being outright launched. This appears to directly inhibit the ability for lunges to get you to your target in a similar manner. It also applies to water, because swimming is basically an extension of flight.
When this happens while you're in ground-based combat, it's a big more problematic. Many suggest that hopping before a lunge or tapping forward will help increase its success.

Hmm, all right, I'll give it a try. Hopefully that'll work, because it's really annoying to try and lunge to get in melee range and not move from where I'm standing.

Unfortunately, you'll never have much luck using lunges with flight active, but they should work fairly reliably during ground combat. I've never had much trouble with them myself, but that must be because I play with a very movement-active style (thanks to my Nostromo) which may innately lend itself toward the "tap forward" and "hop" workarounds without me being aware of it.

The lunges not moving you to your target has been an issue for a while now unfortunately. The best workaround I've found is to jump before using a lunge.

Yeah, I've gotten to the point where I just turn flight off when I enter a Mission. I tempted to retcon out of it (despite it fitting my character) and picking up athletics instead, and then picking up flight at 35, just because I hate moving so slowly during missions (also, because it makes all those enrage stacks I spent so much work building go away.) Strangely, it always seems to work (with or without flight) when I'm lunging upward at something (usually because Roomsweeper knocked it onto a windowsill or something. ;D )
